Mental Wellness as Part of Whole Person Care

Your emotional well-being affects how you think, feel, sleep, work, connect with others, and care for your physical health. Family First Wellness, a DBA of Blakeney Family Practice, PLLC, provides primary care-based mental wellness services for adolescents, adults, and older adults ages 10 and older.

Our providers take time to listen, understand what you are experiencing, and evaluate how your emotional and physical health may be connected. We offer screening, assessment, education, early intervention, and ongoing management for appropriate mental wellness concerns within the primary care setting.

What We Can Assess and Manage

Our primary care providers can evaluate and help manage common emotional, behavioral, and lifestyle concerns, including:

Stress and feeling overwhelmed
Anxiety, excessive worry, or nervousness
Mild to moderate symptoms of depression
Changes in mood, motivation, or energy
Difficulty sleeping or changes in sleep patterns
Grief, loss, and major life transitions
Difficulty concentrating or staying focused
Emotional concerns related to school, work, caregiving, family, or relationships
Burnout and difficulty maintaining a healthy work-life balance
Behavioral or emotional changes noticed by the patient or family
Mental wellness concerns associated with chronic medical conditions
Side effects or concerns related to medications used for conditions managed in primary care.

The care provided will depend on the patient’s age, symptoms, medical history, level of need, and whether the concern can be safely and appropriately managed within primary care.

Services may include age-appropriate screening, primary care assessment, individualized care planning, medication evaluation and monitoring when clinically appropriate, education, brief supportive interventions, follow-up, and referrals.

Services are available for patients ages 10 and older, subject to clinical need, location, licensure, and scope of practice.
