At Blakeney Family Practice, PLLC, we believe patients should understand their financial responsibilities before receiving care. Our team is committed to providing clear information about insurance participation, self-pay options, and anticipated costs so you can make informed healthcare decisions.

Insurance

Blakeney Family Practice accepts select commercial insurance plans. Because participation may vary by plan, service, provider, and practice location, patients should contact our office before scheduling to confirm whether their insurance is currently accepted.

Please have the following information available when contacting us:

  • Insurance company and plan name

  • Member identification number

  • Group number, if applicable

  • Policyholder’s name and date of birth

  • Reason for the requested appointment

  • Preferred location: North Carolina, South Carolina, or virtual telehealth

Our team may assist with verifying eligibility and available benefits as a courtesy. However, verification of benefits is not a guarantee that an insurance company will pay for a service. Final coverage decisions are made by the insurance carrier after the claim is processed.


Patients are encouraged to contact their insurance company directly to confirm:

  • Whether Blakeney Family Practice is in network

  • Primary care and telehealth benefits

  • Copay, coinsurance, and deductible requirements

  • Referral or prior-authorization requirements

  • Coverage for laboratory testing, medications, and additional services

Patient Financial Responsibility

Copayments and known patient-responsibility amounts are due at the time of service. Patients remain responsible for deductibles, coinsurance, noncovered services, and charges denied by their insurance plan when permitted by law.

Some services—including laboratory testing, diagnostic imaging, medications, specialist care, and services performed by outside organizations—may be billed separately by the company or provider performing the service.

Self-Pay Options

Self-pay services are available for patients who:

  • Do not have health insurance

  • Prefer not to use their insurance

  • Have a plan we do not currently accept

  • Need a service that is not covered by their plan

Self-pay pricing is based on the type and complexity of the visit and the services provided. Please contact our office for current pricing before scheduling. Additional charges may apply when laboratory testing, procedures, forms, supplies, or other services are required.

HSA and FSA Payments

Eligible healthcare services may be paid for using a Health Savings Account or Flexible Spending Account card. Coverage and reimbursement requirements vary by plan. Patients should consult their HSA or FSA administrator to confirm whether a particular service qualifies as an eligible medical expense.

Itemized receipts may be provided upon request.

Good Faith Estimates

Patients who do not have insurance or who choose not to use insurance may request a written Good Faith Estimate of the expected charges for scheduled services. The estimate is not a bill or contract and may not include services that could not reasonably be anticipated before the visit.


Additional information about Good Faith Estimates and federal patient billing protections is available through the Centers for Medicare & Medicaid Services.

Virtual Telehealth Services

Insurance coverage for virtual telehealth visits varies by insurance plan, patient location, and type of service. Before scheduling, patients should confirm that their insurance plan covers telehealth services provided by Blakeney Family Practice.

At the time of the appointment, patients must be physically located in a state where the treating provider is authorized to practice.

Payment Policy

Blakeney Family Practice accepts approved electronic payment methods, including major credit and debit cards. Payment arrangements should be discussed with the practice before services are provided.

Cancellation, rescheduling, late-arrival, and missed-appointment policies will be disclosed during scheduling and included in the applicable patient financial agreement.
